Registration fees are applicable to all physicians attending only one day or several days of the CME sessions. Please pre-register so that arrangements may be made for your participation in the meeting. | |

For a full refund of registration and/or activity fees: Cancel in writing by 5:00 p.m., Friday, August 8. Fax your cancellation to fax # 512-370-1655 or e-mail to chris@tsa.org | |

For a partial refund: Cancel between August 9 - 19, there is a partial refund of $75 for registration. There is no refund of the social activities fees. | |
|
No Refund: On or after August 20, there is no refund for the registration fee or social activities fees. | |
